Consent Form for Participation in a Research Questionnaire

The objective of this questionnaire is to understand the current situation, challenges, and ethical dimensions related to the use of Large Language Models (LLMs) and Generative Artificial Intelligence (Generative AI), such as ChatGPT, within your institution. This study is part of the EU-funded project entitled “ADMIT: generative Ai anD large language Models In higher educaTion,” which aims to develop ethical guidelines for the academic community.

Your participation will take place through a dedicated web application. You will be asked to answer a 24-item questionnaire (lasting 10-15 minutes). Upon completion of your submission, the application will provide you with immediate feedback and personalized recommendations regarding the ethical use of AI tools in your own practices.

Your participation in this study is strictly voluntary, and you may withdraw at any time without any penalties. In alignment with the study’s objectives, data will be collected via an online questionnaire. Completing the questionnaire will take approximately 10-15 minutes. Rest assured that all responses will be treated with the utmost confidentiality. You are not required to provide any information that could reveal your identity. To ensure maximum security and confidentiality, the data will be stored in a secure repository using robust encryption methods. Access to this data will be strictly controlled, limited only to authorized partners of this research project, and managed through secure access controls and authentication mechanisms. The data will be retained for the duration of the project, after which it will be securely deleted. Under no circumstances will the data be shared with third parties, upholding our commitment to participant privacy and data security.

No part of the data collection process will include questions or requests that might cause you discomfort. However, if at any point during your participation you feel uncomfortable, you have the option to withdraw from the study by exiting the application. In the event of your withdrawal, all data collected from you will be immediately deleted and discarded.
